feat: split README and INSTALL, add setup.sh
- Split README.md (overview) and INSTALL.md (install for agents)
- Add setup.sh (reference only)
- Unify author to ntzyz <i@ntzyz.io>
- Add Windows PowerShell equivalents and reuse tip
